TALK DM – Taking Action Through Learning and Knowledge Management –
Diabetes Mellitus: Improving Health Care through Communication & Information
TalkDM is funded under a grant from the National Institute of
Diabetes and Digestive and Kidney Diseases (NIDDK). As a two-year
long pilot and planning study, the overall purpose of the grant is to
evaluate the feasibility of using Knowledge Management as an
innovative framework to enhance primary care practices’ adoption and
sustainability of patient self-management support, including
Motivational Interviewing and related counseling and behavior change
approaches and patient tools, in order to improve glycemic, blood
pressure and lipid control in patients with type 2 Diabetes Mellitus.
The research is being conducted in three family medicine practices in
the state of New Jersey, USA, involving clinicians, staff, and
patients; it is in the final stages.
Researchers are:
– Refining materials and methods such as Shared Care Planning and
preparing a manual of operations,
– Collecting preliminary data to estimate effect sizes, sample size
for the larger study,
– Testing the efficacy of using Knowledge Management techniques to
improve knowledge sharing between staff in primary care practices,
especially through patient-physician developed shared care plans, and
– Determining methods and processes needed to translate the proven
Motivational Interviewing and Patient Self Management support
counseling into practice
